Description

The short form of the Centers for Disease Control and Prevention's Healthy Days Measures (CDC-HRQOL-4; HRQOL=health-related quality of life) is a sub-module of the CDC-HRQOL-14 and contains four questions that respondents can use to assess their own physical and mental health. They relate to four key areas:

  • Self-rated general health,
  • Number of days during the past 30 days when physical health was not good
  • Number of days during the past 30 days when mental health was not good,
  • Number of days during the past 30 days on which usual activity was restricted due to poor physical or mental health
